By accessing this website and/or engaging in services provided by Fortitude Mental Health Group Professional Nursing Corporation (“Practice”), including scheduling, intake, consents, telehealth services, or communication, you agree to these Terms and Conditions and all applicable federal and California laws.

If you do not agree, you should not use this website or services.

Content on this website is provided for educational and informational purposes only and is not intended as medical advice, diagnosis, or treatment.

Use of this website does not establish a provider–patient relationship and does not replace professional medical advice, diagnosis, or treatment.

 

3. No Provider–Patient Relationship

Use of this website—including submitting forms, emails, or scheduling requests—does not establish a provider–patient relationship.

A provider–patient relationship is established only after:

  • Completion of required intake procedures through IntakeQ (PracticeQ platform), and
  • Mutual agreement to begin treatment

 

4. Scope of Services

The Practice provides outpatient mental health services including evaluation, diagnosis, treatment, and care coordination.

Services may be delivered in-person or via telehealth.

 

5. No Emergency Services

The Practice does not provide emergency or 24-hour services.

Use of website, email, portal messaging, or telehealth services is not appropriate for urgent or emergency situations.

If you are experiencing an emergency:

  • Call 911
  • Go to the nearest emergency department
  • Call or text 988 (Suicide & Crisis Lifeline)

 

6. Communication Policy

Permitted Communication Methods

  • Phone
  • Email
  • Secure portal messaging (IntakeQ platform)
 

Clinical communication must occur through appropriate channels.

Text Messaging Policy (Strictly Limited)

  • Used for appointment reminders only
  • Not used for clinical communication
  • Not appropriate for emergencies
 

By providing your phone number, you consent to receive appointment reminder texts in accordance with applicable laws, including the Telephone Consumer Protection Act (TCPA).

 

7. Website Communications

Communication through website forms or general email:

  • May not be secure
  • Should not include sensitive medical information
 

Submitting information through this website:

  • Does not establish care
  • Does not create a provider–patient relationship

8. Third-Party Platforms (IntakeQ / PracticeQ)

The Practice utilizes third-party platforms, including IntakeQ (PracticeQ platform), to facilitate services such as scheduling, intake forms, secure messaging, telehealth, and electronic documentation.

Use of the IntakeQ (PracticeQ platform) and any other third-party services may be subject to the terms and conditions and privacy policies of those providers.

The Practice does not control and is not responsible for the performance, security, content, or practices of third-party platforms.

By using third-party platforms in connection with services, you acknowledge that your use of those platforms is subject to their respective terms and policies, and you agree to comply with them. You are encouraged to review the applicable terms and policies of any third-party services you use.

 

9. Telehealth Services

The Practice may provide clinical services via telehealth.

By participating, you acknowledge:

  • Telehealth differs from in-person care
  • Technical issues may occur
  • Privacy risks exist despite safeguards

You agree to:

  • Participate from a private setting
  • Provide your physical location at each session
  • Be located in a jurisdiction where the provider is licensed (including California)

By participating in telehealth services, you acknowledge and agree to the terms outlined in the separate Telehealth Consent Form.

 

10. Patient Responsibilities

You agree to:

  • Provide accurate and complete information
  • Maintain updated contact information
  • Use appropriate communication channels
  • Comply with telehealth requirements
  • Attend or cancel appointments as required
 

11. Privacy and Confidentiality

Your information is protected under applicable laws including:

  • HIPAA
  • California Confidentiality of Medical Information Act (CMIA)
 

You will be provided with a separate Notice of Privacy Practices.

No system can guarantee complete security of information.

 

12. Electronic Communication Risks

By using electronic communication, you acknowledge:

  • Security risks exist
  • Delays may occur
  • These methods are not appropriate for emergencies
 

13. California Privacy Rights

California residents may have rights under applicable laws, including:

  • Access
  • Correction
  • Deletion (subject to legal limitations)
 

Certain medical information may be governed by HIPAA and CMIA.
